To assist you in completing an online application for children
coming to the UK for adoption we have written the following guide.
Section 1. About You

Enter the given name of the child

Enter the family name

Date of birth should entered DAY MONTH YEAR. If you do not know the date of
birth enter "1 Jan 00"

Which country was the child born in

The country of nationality has already been completed for you

If you do not know the date of arrival in the UK choose todays date
Section 1. About you
-
We understand that you may not always have the information about the biological
parents. But, you will not be allowed to move forward with the application
unless you populate the boxes with text. If you do not have the information
simply enter "not known"
Section 1. Where do you live
-
These questions refer to the child. You may use the child's home address,
orphanage or agency acting on behalf of the adoption

If you do not know the information requested enter "not known"

Contact details are important but it is you we will wish to contact if
necessary so check NO if you are not living with the child
Section 1. Contact Address

What we need here is your (the adopting parent/s) address. We may need to
contact you. Typing "not known" in these boxes may delay your application
Section 2. Passport Information

We understand that the passport is not always available or in some instances
not yet issued

At passport number type "00000"

At date of issue select today's date

Date of expiry, select tomorrow's date Place of issue, type "not yet issued"
Section 3. About your sponsor

In this section we need to know about you - the adoptive parent. It doesn't
matter which adoptive parent completes this section
Section 3. (page 2) About your sponsor

Are you financially responsible for anyone else, children, spouse other family
member etc. If you - the adoptive parent - have been married before or at this
time check YES
Section 3. (page 3) About your sponsor

List the people you are currently responsible for. If you have none Type "no
dependants"
Section 3. (page 4) About your sponsor

In this box you should give the details of your spouse/partner. Include full
name, date of birth, passport details, and address (if known) and relationship
Section 4. Your circumstances in the UK
-
This section helps us to determine what the child's accommodation will be like
in the UK. Where will the child live, how many rooms etc. It is not necessary
to complete the e-mail address section
Section 5. Your sponsors circumstances in the UK
-
Section 5 is a simple YES/NO choice, the sponsor is the adoptive parent.
Depending on your choices you may be asked to complete further Information.
Remember that all fields should be completed
Section 6. Relationships

This question refers to the person being adopted, the majority of adoption
cases are generally single but we still need you to select the appropriate
relationship
Section 8. Other family members

If the adoptive child has living family members select YES and continue

If you selected YES to other family members you will now have to choose the
number. If you do not know, select 1
Section 8. Other family members (Person 1)

Remember, you must complete all the fields. If you do not know the answers in
this section you will have to enter the following:

For name, surname and country of residence type "not known"

For date of birth choose today's date

Relationship to you, choose "brother"

Are they: choose single
Section 9. Immigration History

The law requires that this section be completed by all applicants. Read each
question and select YES or No
Review your application

At this section you can review your answers and if necessary make any changes.
Once you are happy with your choices click on NEXT at the bottom of the page
Declaration

The adoptive parent should type their name here. At the end of the process you
will have an opportunity to print your completed application form. You should
print a copy of your form and sign it
Finalise your application
-
You now need to decide how you will be submitting the application. Will you be
sending it by mail or applying in person. You should note that some offices
will require you to book an appointment if you choose to submit in person

You must also decide whether or not to pay online
